{PARTY}’S REQUEST TO COURT REPORTER
TO MAKE A FULL RECORD & PRESERVE ALL NOTES
{Party} asks the court reporter to make a full record of all hearings and to preserve {his/her} notes and recordings from all proceedings in this case.
A. Introduction
1. Plaintiff is {name}; defendant is {name}
2. A copy of this request is served on the official court reporter, {name}, and a copy is filed in the records of this case.
B. Make a Full Record
3. A court reporter is required to make a full record of the proceedings, unless excused by the parties’ agreement. Thus, a party is not required to request that a court reporter attend hearings or the trial and transcribe the proceedings. However, to preserve error, {party} makes this written request and serves a copy on the court reporter.
4. {Party} requests that the court reporter make a full record of all hearings in this case. That includes the following proceedings: all pretrial hearings at which evidence is presented, the entire voir dire, all bench conferences, all evidence presented and objections made during hearings and trial, all orally recorded testimony played during trial, the charge conference, the jury argument, the hearing to receive the jury’s verdict, and all post-trial hearings.
C. Preserve the Notes
5. On request, a court reporter is required to preserve the notes from a hearing or trial for three years from the date they were taken. When the proceedings are recorded by tape recording, the court recorder has a duty to preserve the tapes and the recorder’s logs for at least three years.
6. {Party} requests that the court reporter preserve the shorthand notes and any recordings of all hearings and the trial in this case for at least three years.
